Project 95: Empowering Health, Ending HIV

The CareFirst Foundation is thrilled to introduce “Project 95,” a groundbreaking initiative that tackles the HIV epidemic with a holistic approach to healthcare. With generous support from the Health Council of South Florida and federal funding from the End the HIV Epidemic initiative, “Project 95” not only focuses on HIV prevention and treatment but also addresses broader health disparities and social determinants of health. This comprehensive strategy empowers individuals to take control of their physical, mental, and emotional health, fostering a healthier, more resilient community.

A Holistic Healthcare Model

“Project 95” adopts a multifaceted approach to healthcare that goes beyond HIV prevention and treatment. By addressing the root causes of health disparities and recognizing the impact of social determinants of health, the initiative ensures a more equitable and inclusive healthcare system. Key elements of this comprehensive approach include:

  • Linkage to Healthcare and Support Services: At the forefront of “Project 95” is the seamless connection of individuals to healthcare and other essential services. This ensures that individuals have access to the full spectrum of care they need, from medical treatment to social support systems.
  • Onsite Rapid Testing and Medical Services: Utilizing mobile units, “Project 95” provides onsite rapid HIV, STD, and STI testing, bringing critical services directly to the community. These mobile units are staffed by dedicated professionals, including Nurse Practitioners, who offer a range of medical services beyond just testing.
  • Addressing Social Determinants of Health: Recognizing that health is influenced by a variety of factors, “Project 95” works to address social determinants of health, such as housing, education, employment, and access to nutritious food. By tackling these issues, the initiative helps create a healthier environment for everyone.

Empowering Individuals Through Integrated Care

Central to “Project 95” is the empowerment of individuals. By providing comprehensive healthcare services and addressing broader health disparities, the initiative helps individuals feel in control of their health. This integrated care model includes:

  • HIV/STD/STI Testing and Treatment: Regular testing and prompt treatment are crucial components of “Project 95.” The initiative’s mobile units have reached thousands of community members in Miami-Dade County, offering convenient and confidential testing services.
  • Mental and Emotional Health Support: Understanding the importance of mental and emotional well-being, “Project 95” offers counseling and support services to help individuals cope with the challenges of living with or being at risk for HIV.
  • Access to Prevention Tools: The initiative ensures that condoms, PrEP, and PEP are readily available to those who need them, empowering individuals to take proactive steps in preventing HIV.

Achievements and Future Goals

In its four years of operation, “Project 95” has made remarkable strides in improving public health in Miami-Dade County. The initiative’s comprehensive care model has reached thousands, providing critical services and support to those in need. Looking ahead, “Project 95” is poised to make even more enduring contributions to public health and community well-being.

Milestones and Impact

  • Thousands Reached: Through its mobile units and community outreach, “Project 95” has reached thousands of individuals, providing essential testing, treatment, and support services.
  • Innovative Care Models: By implementing a care model that integrates HIV/STD/STI testing with broader healthcare services, the initiative has set a new standard for comprehensive care.
  • Empowered Communities: Through education, resources, and support, “Project 95” has empowered individuals and communities to take control of their health and well-being.

Looking Ahead

As “Project 95” continues through 2025, the initiative is committed to expanding its reach and impact. By building on its successes and addressing emerging health challenges, “Project 95” aims to create a lasting legacy of improved health and well-being in South Florida and beyond.
